BECOMING A PATIENT WITH EBPG

  1. Complete our Online Prospective Patient Form
  2. Schedule an Appointment with EBPG Staff via Phone
  3. Complete Online New Patient Packet

To see an EBPG doctor, please complete our Prospective Patient Form (see link below). This allows us to verify your insurance benefits to make sure you are covered for a visit. We also like to check with the doctors to make sure they can provide the services you are looking for. EBPG staff will then contact you by phone to schedule your first appointment. Our priority is your health and well-being, and if EBPG is not a good fit for your needs, we suggest you reach out to your insurance company for a list of providers in-network.

Once an appointment is scheduled, we ask for our patient to complete our New Patient Paperwork (see links below). There is a fair amount of information in these questionnaires, so please allow time. We ask for you to complete this before your appointment so that you can spend more time with your physician and less time in the waiting room filling paperwork. 

Please try to keep this appointment as we schedule a fair amount of time and last-minute cancellations are hard for us to manage. If you need to cancel this appointment, please give us a 3-day notice. Without proper notice, we will be unable to offer you a new appointment. You can reach us by phone from 10 am-4 pm, Monday-Thursday. Outside of those hours, please send us an email to ebpgoffice@comcast.net. As a courtesy, we email an appointment reminder to assist your scheduling.
